If employed by a company:
* Name, address and telephone numbers of all employers for past two years
* W-2′s for the past two years
* Most recent pay stub from employer (s)
If self employed:
* Past two years tax returns
* Current Balance Sheet and year to date Income
* Statements for business owned
All Applicants:
* Name, address and account numbers for all checking, savings and investment accounts
* Most recent bank statements on all accounts
* Closing statement on sale of prior home (if applicable)
* Account number (s), monthly payments and approximate balances for all debts
